- A kernel bug in the 6.11 NVMe driver triggered under a specific IO pattern.
- What changed
- Fleet-wide rollback to 6.10 LTS, then a staged upgrade to 6.12 after four weeks of soak testing.

- No customer impact. Two drives failed within an hour in the same RAID-10 array.
- Cause
- A bad drive batch. The array survived because the failures were in different mirror pairs — luck, not design.
- What changed
- Entire batch replaced proactively across all locations. Drive sourcing now deliberately mixes manufacturing lots.

Utility failure with failed generator transfer

- Cause
- Utility power failed and the automatic transfer switch did not engage. UPS carried the load for 11 minutes before depletion.

- Transfer switches now tested monthly under full load rather than quarterly at idle. Second generator commissioned.

- A line card failed and the spare was held at a different facility.
- What changed
